To submit your nomination(s) create a Word document that includes both the narrative and nominee's photo. Save it to your local computer using the nominee's name. Then browse and select it to upload to the web server.
If you have supporting materials, i.e., testimonials etc., scan and save as a PDF file(s) (limit of 3), using the nominee's name, then upload and submit.
